Due to the Privacy Act, information is no longer available for us to include spouse and dependents on your new 2006 Employees Club Card. If you need a card for your spouse, you will need to use the following form or personally make a request at the CEC East or West, W-7 or Pawnee Stores.

Please allow three days for your request to be processed. If you misplace your cards and you would like to replace them, simply complete the form below marking the "Replacement Card(s)" option. There will be a replacement charge of $2.00.
